Problem with Rom Manager after 4.0.4
I unlocked and rooted my phone using Wug's Toolkit. I downloaded 4.0.4 and installed it using ROM Manager. Before doing so, I made a back-up of my stock 4.0.2. 4.0.4 is up an running fine (at least so far). My problem is that now ROM Manager doesn't work. For example: If I try to Flash ClockworkMod Recovery, I get an error stating ""An error occurred while attempting to run privileged commands!" If I try to restore my backup 4.0.2, and say "OK" to "are you sure", nothing happens. It acts just like I had selected "Cancel" rather than "OK." I get the same error message if I try to "Fix Permissions." I have tried uninstalling/reinstalling to no avail. Any help would be greatly appreciated.

Sounds like you lost root when you installed 4.0.4. But here's the su.zip you need to manually flash in the bootloader recovery mode to gain root back. (No wipes just flash) I don't think Wug has incorporated it in his toolkit yet maybe in version 1.4 he will But once u do that reboot device then re-flash clockwork recovery in rom manager. I am having the same problem. I was rooted and then upgraded to 4.0.4. Now I am not rooted so I tried root again. I used the Wugfresh Root Toolkit v1.2.2 this time around to try to root again. I replaced the su.zip file with the one you posted before rooting. Everything seemed to work but when I go to ROM Manager to Flash the Clockwork Mod Recovery it freezes and says "an error occurred while attempting to run privileged commands!" ROM Manager fixed permissions doesn't work either. If I go into Titanium Backup it says I'm not rooted. Suepr User also says the phone is not rooted.
I'm not sure that you can just replace the su.zip in Wug's toolkit, but if you boot into clockworkmod and flash the su.zip 99problemz linked above, it should restore your root on 4.0.4.
Still Can't get root
I got it fixed!!! Must have WugFresh Galaxy Nexus Toolkit.
Restored a backup of stock ICS4.0.2 where it was temporarily rooted.
Ran wugs root to ensure it was permanently rooted then Flashed ClockWorkMod Recovery.
Updated to ICS4.0.4
At this point Recovery Mode from Rom Manager will not work, can't Flash ClockworkModRecovery either, lost root permissions, can't power off and from the bootloader get into Recovery Mode (get dead android with exclamation point).
The work around is to:
In the android-sdk-windows/platform tools folder replace the su.zip (1,100kb) with the su.zip (679kb). This is the file the Toolkit will flash.
Run the Root tool again on Wugfresh and then Flash ClockWorkMod Recovery.
Back that ROM up!
Boy I'm glad I didn't try this then I would have been screwed thanks for your post I'm a amateur at this my phone would have been toast

I still have the problem not being rooted, and I followed the steps, downloaded the file and rooted it with the toolkit 1.3. I don't really know what to do anymore?
Are you on version 4.0.4 of Android?
