What do you mean by a factory reset doesn't seem to work? Were you rooted? What were you doing prior to this happening? What steps have you tried to fix it?

Need a little more info to help you better...

Which recovery do you have? Is it clockworkmod recovery or stock recovery?

If clockworkmod recovery, restore a backup or reflash the ROM and wipe data using the instructions from the thread where you downloaded it.

If stock recovery, you'll want to flash clockworkmod by booting into bootloader mode and following steps 1, 2, 7, and 8 from this guide...[HOW TO] Root the VZW Galaxy Nexus 4G LTE, Flash a Custom Recovery and Make a Backup

You can skip steps 1 and 2 if you already have the clockworkmod .img file

What method did you use to root & unlock? As long as you have all the drivers for your computer to recognize your phone you should be in good shape. If worse comes to worse, just flash back to stock. Let us know how it goes & if you need help with anything.

Make sure you're using GSM (yakju, maguro) ROMs. LTE (mysid, toro) ROMs aren't compatible with the GSM phones. Lots of information here assumes that you're using the LTE version.

You can get the factory images over here:
    https://code.google.com/android/nexus/images.html

    Specifically this one:
    https://dl.google.com/dl/android/aos...y-89fccaac.tgz

If you're on Windows you can extract it with 7zip:
    7-Zip

Inside the tgz you'll find image-yakju-icl53f.zip . Extract the contents of the zip to get system.img .

Start up to the bootloader by holding vol+, vol-, and power until you feel the bump, then flash the stock system image like this:
    fastboot flash system system.img

That should get you back to the stock ROM but still have CWMR installed. If you get stuck let us know, or if you get it working we can then help you get your custom ROM installed.

IF you flash the system will you lose data such as phone contacts and photos?
